Toronto, which had a wildfire spread of SARS due to sloppy medical procedures, stopped it dead with quaranteens. So I don't think the problem is total numbers, I think a contributing factor is population density. But any city has a population density far above anything expected for the world at large this century. By the way, the WHO has brought its future population estimates down to acknowledge the spread of the "demographic discontinuity" to the third world.
